About this package

What it does

Employee pulse survey is a monthly engagement check built only from UserTasks features and the official form controls. The survey page has six steps with a progress bar and a review step: four Likert scale matrices (wellbeing, manager & team, growth, workload; agree / disagree with N/A), a 0-10 Slider for how likely the employee is to recommend working here (eNPS), a heart Rating for mood, the department, an "Answer anonymously" option and a Rich text box for one thing to change. The manager Person field only shows when the answer is not anonymous.

Every answer starts the Pulse response workflow. Set Variable expressions average each section (ignoring N/A) and turn all the answers into an engagement score out of 100; a decision table maps engagement, mood and eNPS to Thriving, Okay or At risk and decides whether a follow-up is needed. The answer is saved in the Pulse responses data table, without a name when anonymous; the table ships with 30 sample answers across six departments and three monthly rounds.

Named at-risk answers become a private follow-up task for the manager (or the person who installed the package when no manager was picked) with an SLA, a reminder and escalation. The task shows the scores on an Info card and a Progress bar, and records an action plan in a Table control (action, owner, due date) plus conversation notes, which are emailed to the employee. Anonymous at-risk answers never create a personal task: they are logged and counted in the HR aggregate.

The Pulse results page is a dashboard: Chart controls for average engagement by department, eNPS by round and the risk-band split from the data table, Saved report controls for section averages and open follow-ups, an Info card explaining the scores, and a button that emails a round summary.

Starts from a form

Starts from a form page: share the Employee pulse survey page (/f/<id>) with signed-in employees each month, or submit it from your own app through the forms API.

Someone fills in a form to start it: the workflow start form (Run dialog in the app, or your own portal posting to the API) or a standalone form page (/f/<id>) whose Form submitted trigger starts the workflow. People complete User Task forms along the way.

How forms work

The flow

Step by step

  1. 1

    Employee answers

Six steps with a progress bar: four Likert sections, the eNPS slider, mood, department and one thing to change, then the anonymous choice and an optional manager, with a review step.

  2. 2

    Score

Set Variable expressions average each section (N/A ignored) and work out an engagement score out of 100 and eNPS points.

  3. 3

    Band

A decision table maps engagement, mood and eNPS to Thriving, Okay or At risk and decides whether a follow-up is needed and how fast.

  4. 4

    Save

The answer goes into Pulse responses with its round and department; anonymous answers are stored without a name.

  5. 5

    Follow up

Named at-risk answers get a manager task with an SLA, the scores on an Info card and Progress bar, and an action plan table that is emailed to the employee. Anonymous ones are logged for the HR aggregate only.

  6. 6

    Watch the trend

The Pulse results page charts engagement by department and eNPS by round, shows section averages and open follow-ups, and emails a round summary.

Pulse response — An employee answers the pulse survey: section scores and an engagement score are worked out, a decision table sets the risk band, the answer is saved (without a name when anonymous), and named at-risk answers get a manager follow-up with an SLA and an action plan. The same canvas you get in the designer after installing.

Sub-workflow: Pulse round summary

From the Pulse results page: averages engagement, eNPS and the four sections for a survey round in the Pulse responses table, counts the risk bands and emails the summary to whoever asked.

Forms

The start form and the task forms

People start it from a form page (/f/<id>) with its own header, theme and buttons; submitting it starts the workflow. User Tasks show their fields and outcome buttons to the assignee.

Form controls

9 official form controls, installed with it

The forms use these controls from the gallery. They come inside the package and install into Extensibility → Form controls, sandboxed and signed like every official control: nothing else to download.

Requirements

What you connect it to

Credentials never travel in a package. The install wizard asks for what your environment must provide, or installs it incomplete until you finish setup.

Needs

  • Email (optional) — Email delivery configured for the instance (EMAIL_PROVIDER), used for notifications.

After installing

  1. Nothing to configure: install it and open Forms → Employee pulse survey (share its /f/ link each month) and Forms → Pulse results.
  2. The Likert scale, Slider, Rating, Rich text, Progress, Info card, Table, Chart and Saved report controls install with the package (Extensibility → Form controls).
  3. Follow-ups go to the manager the employee picks; without one they go to the person who installed the package (an HR partner, for example).
  4. Edit the Engagement risk band decision table to change the thresholds or follow-up days; delete the sample rows in Pulse responses when you go live.
  5. Optional: apply the "Pulse responses: 18 months" retention policy, and set EMAIL_PROVIDER so the thank-you, plan and summary emails are delivered (the console provider logs them).

Asked for on install

Nothing: it installs complete.

Install

Install it in your workspace

Works on UserTasks cloud and on any self-hosted instance. Installing creates the assets above; nothing runs until you complete setup and start it.

In the app

  1. Download employee-pulse-survey-1.0.0.utpkg.
  2. Open Automations → Packages and drag the file onto the page (or choose Import package). Browse gallery there lists every gallery package without downloading by hand.
  3. Pick the file. The wizard verifies the checksum, every file hash and the signature, and shows the readme.
  4. Review the changes (what is created) and the data rows.
  5. Fill in the requirements, or tick Keep incomplete and finish later with Complete setup.
  6. Done: the workflow is published and active once setup is complete.

With the CLI

shell
curl -fLO https://usertasks.com/gallery/employee-pulse-survey-1.0.0.utpkg
usertasks packages inspect employee-pulse-survey-1.0.0.utpkg # offline: checksum, hashes, signature
